For patients considering platelet-rich plasma (PRP), bone marrow concentrate (BMC), or other regenerative medicine procedures, one question is often overlooked: Does it matter how the treatment is performed? Research increasingly suggests the answer is yes.

Regenerative medicine is designed to deliver biologically active cells and growth factors directly to injured tissues. However, these treatments are only as effective as the physician’s ability to accurately identify the damaged structure and precisely place the injectate where healing is needed.

Regenerative medicine is more than simply “getting an injection”

Many musculoskeletal conditions involve much more than cartilage damage or arthritis visible on an X-ray.

A systematic review published in British Medical Bulletin (1) concluded that ultrasound guidance significantly improves the accuracy of musculoskeletal injections while reducing procedural complications.

Another review (2) found that ultrasound-guided injections consistently achieved higher placement accuracy than blind injections across multiple joints.

More recently, the European Society of Musculoskeletal Radiology (3) has recommended ultrasound guidance as the preferred technique for many soft tissue and joint interventions because of its ability to improve procedural precision while avoiding nearby nerves and blood vessels.

For regenerative procedures, accurate placement is particularly important because PRP and cell-based therapies rely on delivering biological materials directly into damaged tissue rather than simply reducing inflammation.

Dynamic ultrasound can identify instability

One unique advantage of ultrasound is that it evaluates tissues during movement. Many ligament injuries become apparent only when stress is placed across the joint.

Dynamic imaging can reveal:

  • Ligament laxity
  • Partial tears
  • Tendon subluxation
  • Joint instability
  • Abnormal tissue motion

This information may influence treatment planning and explain pain that static imaging fails to identify.
